It is dense, written in small print, and lists so many potential side effects that patients often dismiss it entirely. Yet that document—the prescribing information, or package insert—contains critical safety data that can make a meaningful difference in how you respond when something feels wrong after starting a new medication.
Adverse drug reactions (ADRs) account for an estimated 125,000 deaths and more than 1 million hospitalizations in the United States each year, according to data published in the Journal of the American Medical Association. Many of these events are preventable—not because the medications are inherently dangerous, but because patients and providers sometimes lack the information or communication tools to identify a reaction before it becomes serious.

Understanding the Spectrum: Not All Side Effects Are Equal
Medical professionals draw an important distinction between side effects and adverse reactions, though the terms are often used interchangeably by patients. An adverse reaction specifically implies harm.

Abruptly stopping certain antidepressants, benzodiazepines, or corticosteroids can trigger withdrawal-like symptoms that patients may misinterpret as a return of their original illness. This is a particularly important consideration for anyone considering stopping a long-term medication without medical guidance.
How to Document a Suspected Reaction Effectively
If you believe you are experiencing an adverse reaction, thorough documentation strengthens your ability to communicate with your provider and contributes to broader pharmacovigilance efforts. Consider keeping a medication reaction log that includes:
- The name, dose, and frequency of every medication and supplement you are taking
- The date and time you first noticed the symptom
- A precise description of the symptom, including location, severity, and whether it is constant or intermittent
- Any changes in your routine, diet, or other medications that coincided with the onset
- Whether the symptom improved or worsened when you took or skipped a dose
This level of detail is invaluable when your provider needs to determine whether the drug, a drug interaction, an underlying condition, or an unrelated factor is responsible for your symptoms.
Reporting to the FDA: MedWatch Explained
The FDA's MedWatch program is the official safety reporting system for adverse events related to medications, vaccines, medical devices, and dietary supplements. Reporting is not only for healthcare professionals—patients and caregivers are explicitly encouraged to submit reports, and consumer submissions have contributed to several significant drug safety actions over the years.

Advocating for Yourself With Your Healthcare Provider

Be specific, not vague. "I feel off" is harder to evaluate than "I have had a persistent headache every morning since I started this medication five days ago."
